She suffered a closed fracture of the femur shaft after a fall, 🔍 **Challenges Faced:** 1. **Difficulty with Positioning:** Proximal femur abduction hindered proper positioning for surgery, prolonging the process. 2. **Limited Equipment:** Insufficient tools, like a 13mm reamer, posed challenges during the procedure. 0.5 mm increment reamers should have been used 3. **Narrow Canal:** she had a narrow intramedullary canal, requiring meticulous handling ( although went on with some shattering at fracture site) 4. **Fracture Complications:** Inserting the nail required considerable force, leading to fracture site shattering and distraction ( had to use back slap hammer after distal locking ) 5. **Protruding Nail:** The nail ended up protruding. 6. **Neck Protection:** Considering her comorbidities, a different nail type might have better protected her neck. 💡 **Management Approach:** - Dynamic locking proximally was implemented, allowing for controlled weight-bearing after 10 days. - Pain-free walking was achieved after three months, despite initial delays in union on xrays - At seven months post-op, she is walking without support, enjoying a pain-free life. **Takeaway:** Every case is unique, and individualized care is paramount for optimal recovery. #OrthopedicSurgery #MedicalCaseStudy #PatientCare #FemurFracture #Orthopedics #medicaleducation #sgsmhospital #itpawareness #femurfracturerecovery #closedreductioninternalfixation #fractureunion"
