An unencrypted USB flash drive, which stored the data, disappeared Nov. 20 from the hospital's emergency department at 5165 McCarty Lane. The flash drive contained spreadsheets with information from emergency department patients dating back to Nov. 1, 2014. The information includes patient names, dates of birth, home telephone numbers, medical record numbers, physician names, diagnoses and dates of service. (link)
