Doctor Dolittle has a few close human friends, including Tommy Stubbins and Matthew Mugg, the Cats'-Meat Man. The animal team includes Polynesia (a parrot), Gub-Gub (a pig), Jip (a dog), Dab-Dab (a duck), Chee-Chee (a monkey), Too-Too (an owl), the Pushmi-pullyu, and a white mouse later named simply "Whitey".[1] Later on, in the 1925 novel Doctor Dolittle's Zoo, Whitey founds (with the doctor's help) the Rat and Mouse Club, whose membership eventually reaches some 5,000 rats and mice.

In 1845, in the small port town of Puddleby-on-the-Marsh, in western England, Matthew Mugg brings his young friend Tommy Stubbins with him when he takes an injured duck to his friend, the eccentric Doctor John Dolittle, who he claims is the best veterinarian in the world because he can talk to animals. A former medical doctor, Dolittle lives with a large menagerie of various creatures, including a talking blue-and-yellow macaw named Polynesia, a chimpanzee named Chee-Chee, a dog named Jip, and a piglet named Gub-Gub. He explains that the many animals he kept in his home created havoc with his human patients, who began to go elsewhere for their medical needs, so his sister, Sarah, who served as his housekeeper, demanded that he dispose of the animals or she would leave, and he chose the animals. Polynesia then revealed that she could speak over 2,000 animal languages and offered to teach Dolittle so he could become an animal doctor. Dolittle says he can now speak nearly 500 animal languages, and he is currently in the process of learning how to speak with sea creatures for an upcoming expedition to search for the legendary Great Pink Sea Snail.

To help Dolittle earn money for his expedition, a friend sends him a rare pushmi-pullyu, which is a creature that looks like a llama with a head on each end of its body and that likes to dance. Dolittle takes the pushmi-pullyu to a nearby circus run by Albert Blossom, and it becomes the star attraction. The doctor talks with a sad seal named Sophie and learns she longs to return to her husband at the North Pole, so he smuggles her out of the circus, disguises her in women's clothing to convey her to the coast, and throws her in the ocean. Some fishermen witness this and have Dolittle arrested for murder.

The film then jumps forward to Dolittle as an adult, married to a woman named Lisa and with two daughters, Charisse and Maya. John works as a doctor and surgeon, and his company is about to be bought by another medical company.

He is helped by his friend and colleague Gene, who sees him sneaking into the hospital. A party is also going on in the hospital, and everyone ends up watching as Dolittle and Gene perform surgery on the tiger, saving his life. Dolittle accepts his gift after this, deciding not to sell the hospital, and becoming both a doctor and veterinarian.

Dolittle proceeds to strap the cage to the top of his car and continues driving down the road. Rodney continues to try and talk to the doctor, but he turns up the radio in an attempt to drown him out.

A visit from EMMA FAIRFAX, the niece of General Bellowes, ends on a sour note, but the mood soon brightens with the arrival of the PUSHMI-PULLYU, a two-headed llama. They set off for Sea Star Island and, after encountering a storm at sea, end up shipwrecked in the very place they were looking for. There the travelers discover STRAIGHT ARROW, the friend who sent the Pushmi-Pullyu to Doctor Dolittle. Straight Arrow explains that Sea Star Island is an animal sanctuary (Save the Animals).

Doctor Dolittle's dream finally comes true with the arrival of the Great Pink Sea Snail to the island. They converse in Escargot and the Snail agrees to take the doctor's friends back home. Doctor Dolittle explains that he will not be returning to Puddleby and the friends share a tearful goodbye before departing in the Snail. Back home, Emma tries to talk some sense into her uncle with the help of the animals (The Voice of Protest). It works and Bellowes declares Doctor Dolittle innocent of any wrongdoing. The cheers of the crowd are interrupted by the triumphant return of Doctor Dolittle to Puddleby-On-The-Marsh (Finale).

He makes a bet that he can beat the bullfighters at their own game. If he does, they have to stop the practice of bullfighting on their island.
