Admission Process

Dear Applicant,

Welcome to the De La Salle Medical and Health Sciences Institute - College of Medicine Admission Space and thank you for your interest in applying to our institution. This will be your first step in fulfilling your dream on becoming a future Lasallian Physician.

Online Application Process

Application Number

The Application Number is an eight (8) digit number issued by the Lasallian Access and Success Office (LASO) after paying the Admissions Processing Fee. The application number will be used to track and monitor your submitted application forms, documentary requirements and recommendation forms. The application number will only be used during the application process in the College of Medicine.

Documentary Requirements

  1. Before accessing the Online Application Form, make sure that you have the following documents available in PDF format.

  • NMAT result

  • Transcript of Records (TOR)/Certification of Grades

  • PSA Certificate of Birth (for Filipino Citizens)

  • Certificate of GWA/GPA

  • Certificate of Graduation

  • Notarized Sworn Statement (download the form here: Sworn Statement Form)


  1. Prepare a digital copy of your recent picture with the following specifications:

  • size: 2x2

  • white background

  • with full name tag (format: Surname, First name, Middle name) at the bottom of the image

  • file should not exceed 10 MB


  1. All documents and digital picture must be saved using your Application Number as the filename. (Ex: 20221234.pdf or 20221234.jpeg)


  1. All documents uploaded in the College of Medicine Admission Space will only be used to evaluate your qualification as a prospective medicine student. Applicants are still required to comply with the submission requirements set by the LASO and the Registrar Office.


Online Application Form

  1. Completely fill-up the Online Application Form. To access, click the link: Online Application Form.


  1. Once completed, the applicant will receive an email notification verifying the receipt of their application including the schedule of interview.


  1. If no email received within 24-48 hours, the applicant must immediately contact the College of Medicine at (046) 481-8000 local 1309


Recommendation Form

The applicant must request from two (2) former faculty preferably the class or research adviser to fill-up the standardized recommendation form. To access the form, click the link: Recommendation Form

Once the form have been completely accomplished, the recommender must send the form via email at dlsmhsicmadmissions@gmail.com with the Applicant's Number as the subject.

Interview Procedures

Only those applicants who were able to submit all the required documents, have completely filled-up the Online Application Form, and were recommended by the chosen advisers, will be subjected for interview.


Interview Schedule

  1. After submitting the Online Application Form, the applicant will receive an email notification verifying the receipt of the application and the details of the interview appointment schedule.

  2. Cancellation or rescheduling of interview appointment is not permitted. Failure of the applicant to appear on the interview appointment schedule will result in disqualification of the application.

  3. In the event that the applicant is encountering technical issues such as poor internet connection, power failure, he/she must notify the College of Medicine thru phone call at (046) 481-8000 loc 1309.


Interview Platform and Venue

  1. Due to the current pandemic situation brought about by Covid 19, all interviews will be conducted virtually using Google Meet.

  2. Applicant is expected to secure a stable internet connection and must choose a venue with minimal environmental noise.
