Marigold Permission Form

For grades 1-7, a permission form must be filled out and submitted to Mrs. Kelly at takelly@rockyview.ab.ca . 

Please print out the library card application by clicking on the "pop-out" button on the top right of the form. You may then print, fill out, then scan and submit the form.

The following are the required fields:
-last name
-first name
-email address (please use school email address)
-do you have a Marigold/TRAC library card?
-parent/guardian signature
