Required Forms

To be considered for the 2024 - 2025 Cheerleading Team, please complete the following forms and submit them to Coach Alyssa at denvernorthcheer@gmail.com by 5/6/24.

NHS Cheerleading Contract

All candidates must read and understand the contract to be considered for the 2023 - 2024 Cheerleading Team.

NHS Cheerleading Tryout Form

All candidates must submit the Tryout Form to be considered for the 2024 - 2025 Cheerleading Team.

Faculty Recommendation Form 

All candidates must have 3 or more Faculty Recommendations submitted on their behalf. The candidate must email the form to 3 or more of their current teachers. Please have the faculty member submit the form electronically.

Recommendations are heavily weighted in your final tryout score. 

Recommendations will be completely confidential and will only be seen by the coaches.

Sports Physical

All candidates must submit a current sports physical provided by their physician to be considered for the 2024 - 2025 Cheerleading Team. Formats can vary.

You may use this form if needed.

Please email your current physical to Coach Alyssa at denvernorthcheer@gmail.com by 5/6/24.

Fall 2023 Semester Final Grades

All candidates must submit a print out of their final Fall Semester grades to be considered for the 2024 - 2025 Cheerleading Team.
