Every year there are 4 bloodthirst born. Everywhere in the world, these guys are feared, as they are craved with blood. (however some hate the taste of flesh and blood)
There is a chance that they are born from birth or diagnosed by growth.
The world is preferably choosing to have bloodthirst killed instead of letting them stay alive so more do not get born.
It has been this way for years.

E'foruim being the main island is divided into 4 islands, E'foruim being main, T'foruim , D'foruim, and Classified as that's only allowed towards governors. This place is swarmed with monsters, Withered angels to bloodthirst, all the same. The president tries her best to keep everything down for the people of E'foruim to keep safe.
However Dr. Gavelyn seems as her ways aren't imfactful within this world that has been created, and sees to stop to continue the studies of what's been haunted and to stop it himself.
Even if that means killing her.
