To enroll in the Sick Leave Bank, print and complete this form. Please review the eligibility requirements, and send your completed form to Karen Lamb at Douglas High School. Enrollment runs from September 15 - October 15th.
Here you can find the form used to apply for use of the Sick Leave Bank. You will be forced to "Make a Copy." Then simply fill out your information, print, sign, and send to Karen Lamb at Douglas High School.
Note: This is not the form to JOIN the Sick Leave Bank; this is used to DRAW from the Sick Leave Bank!
