BSA Release and Health Forms Part A&B Required for participation in Troop Activities

• Parents are welcome to participate, please fill out forms as well

BSA Release form Part C is required for all camping activities longer than a weekend (required for summer camp)

These forms should be handed to the Scoutmaster or the Health & Safety Coordinator, all information is confidential at all times.
