School Indemnification

Precaution and safety measures are put in place by Dave’s Foundation and/or its coaches to ensure the safety of participants.

I hereby confirm and agree that any personal injuries, death arising, or any loss or damage of property from participating in the activities, will not be held liable on Dave’s Foundation and/or its coaches, except for any injuries or death arising caused directly by Dave’s Foundation and/or its coaches’ gross negligence.

Liability Waiver

I hereby consent for my child/myself to partake in the activities and consent to any emergency treatment as necessary. I accept that Dave’s Foundation and its coaches will not be held accountable whatsoever for injury, loss, or damage to my child’s/my health or property whilst participating in any coaching sessions, camps, tournaments, or other activities wherein my child/myself participates in for this term or any future terms. Dave’s Foundation does not provide any insurance coverage for you or your child.

I understand that participating in any activity organised by Dave’s Foundation is at my child’s/own risk. The safety and well-being of the children and other participants are a priority for Dave’s Foundation.

I herewith consent that any photos or videos taken during Dave’s Foundation activities are the sole properties of Dave’s Foundation and may be used for marketing purposes. These photos will not be handed over to any third parties. Dave’s Foundation holds all the rights to use these photos and videos on marketing platforms such as TikTok, Facebook, Instagram, and/or Twitter. If you wish for us not to use photos or videos that include your children/yourself, please write to us at davefoundationschool@gmail.com with the subject line “photographs and videos”.
