Forms

Prior to your very first grooming appointment, you will need to fill out our new client paperwork. If you would like to get ahead of the game and have these filled out before your arrival, you can print the following two forms:

• Client Information Form

• Pet Information Form- you will need one of these for each dog (so if you have multiple dogs, print one for each dog)

You will also need to provide vaccination records for each of your dogs. You will need to have current certificates of the following vaccines:

• Bordetella

• DHPP

• Rabies

You can do any of the following to provide these records:

• Bring originals with you to your first appointment and we will scan them in to our system

• Bring copies with you to your first appointment and we will scan them in to our system

• Scan them yourself and email to dapperdogspremieregrooming@gmail.com prior to your first appointment
