At Customised Forms Online, we create online forms to gather the information you need for your club, class, research, or small business. We create a link between you and your client. You may need an online form to enable your clients to:
register for a class
complete a survey & gather data
register for an event
sign-up for a club membership
We create a form, and you can then circulate the link via email, text, Whatsapp, and social media. If you already have a website we can link the online form to your website. However, it is not necessary to have a website to distribute your forms. Data from the completed forms will be stored on a linked spreadsheet. You can store, print and manipulate the spreadsheet as required.

Please contact us for a competitive quote. You will pay a one-off fee per form regardless of the number of times you distribute it. Once purchased, you will own the form and be responsible for adhering to all the relevant data protection legislation.
