JOB DESCRIPTION

JOB TITLE: Social Worker

Reports directly to: Social Work Supervisor

JOB ROLE: To support clinical internal research projects at the Hospital in relation to patient care among others with specific outputs from the clinical research group.

KEY RESULT AREAS AND ACTIVITIES

Patient and family Assessments:

1. Conduct Socio-Economic Assessments of each patient’s family.

2. Develop a comprehensive understanding of the social service needs of the patient’s family.

3. Provide feedback to the department supervisor on the family’s socio-economic situation for use in determining payment plan for private and adult patients.

4. Continually identify, enroll and assign patients to sponsor organizations including CCHU as a primary sponsor.

5. Together with the department supervisor and finance department, process and follow-up on all outstanding invoices from sponsor organizations, private and adult patients. on monthly basis.

Actively manage a case load of assigned patients/families in the provision of Social Work services offered at the hospital and linking to the community.

1. Provide orientation to all patients on the operations of the hospital and services provided including spiritual ministry, patient sponsorship, patients’ rights and responsibilities, in liaison with the various departments.

2. Provide counseling to patients and families on a wide range of life and health issues (such as child development and wellness, financial sustainability, HIV/Aids counseling, etc.

3. Actively manage and track a case load of assigned parents/families.

4. Complete regular patient home visits and meetings with parent support groups.

5. Follow-up with patients on issues presented through the Assessment process.

6. Follow-up patients who miss medical appointments. Endeavor to do at least one patient follow up per week.

7. Work with your supervisor and finance department to help ensure private patients pay their bills as per the agreed terms.

8. Together with the CURE storyteller, write at least 1 patient story (especially success stories) with photos per month as a resource to draw from when requested.

Community Linking function

1. Obtain patients consent to share their information with sponsor organizations, and CBR and other support organizations.

2. Link patients and their families to CBR and other support organizations in their respective communities or regions.

3. Active involvement with parent support groups in the assigned region.

4. Build relationships with key social services organizations, CBR groups, and other support organizations that are available in the respective regions for linking our patients.

5. Advocacy for vulnerable children / awareness building in the community around children with neurological health conditions (HC, SB, EC, etc.).

Support of the mobile clinic function

1. Ensure smooth running of monthly mobile clinics

2. Work with the IT and Health Information office to obtain patients list for each clinic to be able to establish the expected number of patients, find their files.

3. Coordinate with respective departments to establish the number of staff needed for the clinic.

4. Share annual clinic calendar with the host organizations or hospital, confirm their readiness to host the clinic.

5. Registering Patients as per established protocols Handle all administrative and housekeeping issues (including transport, accommodation, meals and other payments for support hired teams at the clinic

6. Prepare Satellite Clinic report and accountability

7. Share with supervisor and other departments matters that need improvement or other form of action for the next clinic(s).

Reporting function done

1. Submit routine reports documenting various activities

2. Work as a contact person of the hospital and the patient sponsor organization where necessary.

3. Together with the finance department, prepare reports for patients sponsor organizations as per the agreed guidelines.

Spiritual Ministry

1. Participates in the hospital’s Spiritual Ministry programs.
