MESA College Prep Program - Registration Form

You will be asked to input the following information (and more) on the student registration form:

• 10-digit CA SSID - can be obtained from your counselor, a report card, your student portal, or by asking your advisor for assistance

• Home address

• Parent/Guardian name, email, phone number, education level, and occupation

Note: You cannot submit the MCP registration form without completing all required information, and you cannot go back to edit the form once submitted. Any missing or incorrect data will result in a rejected registration or will delay your enrollment process.

Packets may be submitted to your MESA advisor or emailed to coe-mesa@csulb.edu.

The Parent/Guardian Waiver Packets contain the following documents*: (1) MESA Parent Authorization Form (PAF), (2) CSULB YA Form: Liability Release, (3) CSULB YA Form: Photo/Video Release, (4) CSULB YA Form: Emergency Contact, (5) CSULB YA Form: Medical Disclosure

*PUSD Students have additional General Release and Photo Release waivers for Saturday Academies. PUSD students participating in the Saturday Academies must complete the MESA enrollment requirements BY the first Saturday Academy on October 17th.
