MESA College Prep Program - Registration Form

You will be required to input the following information (and more) on the student registration form:

• 10-digit CA SSID - can be obtained from your counselor, a report card, your student portal, or by asking your advisor for assistance

• Parent/Guardian name, email, phone number, education level, and occupation

Please note that you cannot submit the student registration form without all the required information and you cannot go back to edit the form afterward. Any incorrect information will result in a rejected registration and will delay the enrollment process.

Note: Students that are completing registration in class may use the Student Registration Form (Guide) provided in the Parent/Guardian Waiver Packet, to collect the data with a parent before completing the CSULB MESA - Student Registration Form.

The Parent/Guardian Waiver Packet contains the following documents:

• MESA Parent Authorization Form (PAF)

• CSULB YA Form: Liability Release

• CSULB YA Form: Photo/Video Release

• CSULB YA Form: Emergency Contact

• CSULB YA Form: Medical Disclosure

*PUSD Students have additional waiver forms for Saturday Academies and must complete enrollment requirements before or by the first Saturday Academy they attend.
