Bus Form

Costa Mesa High School Volleyball players are required to ride the bus to & from all away volleyball games with their team.

However, we realize that's not always possible with busy schedules.

If a player is NOT able to ride to and/or from the game on the bus:

Please download/complete the 'Bus Form' and have Coach Jacob sign it for you. 

THIS HAS TO BE DONE IN ADVANCE -- NOT ON THE SAME DAY AS THE MATCH.

Players will also need to notify their team coach.
