Cosmic Auto Repair: Service Agreement & Authorization

Thank you for choosing Cosmic Auto Repair (DBA of Cosmic Motorsports, LLC)! Before we begin working on your vehicle, please review the following service agreement. This outlines important information about repair authorizations, warranties, payment terms, and more.

By accepting this estimate or paying the invoice, you authorize Cosmic Auto Repair to perform the described repairs and agree to our full terms at https://sites.google.com/view/cosmicautorepair/home or as outlined below.

Repair Authorization & Terms

I, the undersigned, hereby authorize Cosmic Auto Repair to perform the repair work as described in the provided invoice or estimate, including any necessary parts, materials, and labor. I acknowledge and agree to the following terms:

Liability Waiver

Cosmic Auto Repair is not responsible for loss or damage to the vehicle or any personal property inside the vehicle due to fire, theft, or any other causes beyond its control.

Vehicle Operation Consent

I grant permission to Cosmic Auto Repair and its employees to operate the vehicle on streets, highways, or elsewhere for the purpose of testing, diagnostics, and/or inspection.

Garage Keeper’s Lien

An express garage keeper’s lien is acknowledged on the vehicle to secure the cost of parts and labor related to the repair. In the event of non-payment, legal actions may be taken to enforce the lien.

Storage Policy

Vehicles left over 48 hours after the completion of repairs will incur a $5.00 per day storage fee.

Warranty Terms

Limited Warranty: 6 months or 6,000 miles (whichever comes first)

• This warranty applies only if parts used for the repair are provided by Cosmic Auto Repair.
  
  

• Wear items (e.g., brake pads, belts, clutches) are not covered under warranty.
  
  

• Warranty is voided if the repaired system is tampered with by the customer or a third party.
  
  

• Warranty does not cover user error, accidental damage, or misuse.
  
  

• There is no warranty—express or implied—on customer-supplied parts (if accepted).
  
  

• No warranty is offered on diagnostic visits or recommendations based on diagnostics.
  
  

Payment Terms

• Payment is due in full upon completion of repairs, unless otherwise agreed in writing.
  
  

• A deposit of up to 50% of the estimated repair cost may be required upfront to cover parts.
  
  

• Failure to pay in full may result in enforcement of a garage keeper’s lien, legal collection actions, and additional fees.
  
  

• Final payment must be made before the vehicle is released.
  
  

Acknowledgment & Consent

By signing below, or receiving a copy of this agreement via email, text, or Square electronic invoice/estimate, I confirm that:

• I have read, understood, and agree to all of the terms and conditions listed above.
  
  

• I agree to pay the total balance upon completion of the repairs.
  
  

• I understand that failure to pay may result in a lien and/or legal action.
  
  

• I acknowledge and consent to all terms regarding liability, storage fees, and warranty limitations.
  
  

• I authorize Cosmic Auto Repair to perform the repair(s) described in the estimate/invoice.
  
  

• I understand that digital approval of this estimate or invoice constitutes written authorization under California BAR regulations.
