To access the EMR, open your Internet browser to the following URL:
http://www.core-at.com
Because the EMR is used by athletic trainers across the country, it requires you to specify your clinic/high school, your personal username, and your personal password.
Enter your clinic id in the first field (home symbol)
In the next field, enter your personal username. (person symbol)
Finally, enter your personal password. (lock symbol)
Now, click on “Login”.
The AT Dashboard is the starting point within the EMR, and is where you can access all of the different components within the system.
The navigation menu across the top of the page provides access to the major system components, including the ISC, registering new patients, viewing daily logins, managing public registrations, and running reports.
The Dashboard is divided into sections. The first is a snapshot view of the current system. This includes values for Year To Date Economic Estimates, total currently registered patients, total number of patients with a current injury, total number of draft forms, and total number of overdue patient outcome forms. These snapshots are designed as helpful reminders for the ATs working in the system on a daily basis.
The second section is the Patient’s List. This is where all patients who are registered within the system are listed. You will notice the search box that allows you to type in a patient’s name to search. In addition, you can sort this list by the header rows; patient, current injuries, draft forms, overdue forms, and last visit.
To navigate to files for an individual patient, click on the down arrow next to the “view” button. This will bring up several options for patient.
As you can see, from this menu you have the ability to view the patient’s profile/injuries, add a new injury, add a daily login entry for the patient, edit their profile, or archive the patient. You can also navigate directly to the patient’s profile/injuries by clicking on the “view” button.
The forms list is the last section of the AT Dashboard. In this list are all forms that have been completed within the system.
There are several ways to interact with the forms list. First, you can select to display only forms completed by the AT (injury eval, discharge, etc.), only forms completed by the patient (outcomes), or all forms in the system. This is done using the “AT,” “All,” or “Pat” buttons at the top right. Second, you can sort the forms by status, including draft, completed, overdue, and upcoming. Last, you can search the forms by typing in the type of form (injury demographic, eval, etc.) or typing in the patient’s name.
* Please note – the default view for the patient and form lists includes only unarchived patients and those patients that have been seen or forms that have been completed within the last two months. If you cannot find the patient or form you are looking for, you may need to refresh the patient list using the link at the bottom of the page.
Also, both lists have a default view of 10 entries per page. This can be adjusted at the bottom of the lists.
The Patient Dashboard provides access to all of the documentation for a specific patient. This is where you will enter new injuries, document daily treatments, complete discharge notes, and upload patient files.
To reach a patient’s dashboard from the AT Dashboard:
1) Click on the “view” button for the selected patient, located on the right side of the patient list.
OR
2) Click on the drop down arrow next to the “view” button for a selected patient and select “Patient Profile/Injuries”.
The Patient Dashboard is similar in design to the AT Dashboard. It includes snapshot information for the patient at the top and lists all patient forms related to a specific injury. At the top, you will see an accounting of the patient’s total number of current injuries, the number of forms due soon, the number of overdue forms, and the number of draft forms. In addition, there are the “Actions,” "Baseline," and “Quick Info” buttons.
The action button gives options to Add a new injury, add a daily login for the patient, view uploaded patient files, print the patient’s entire record, login as the patient (to complete outcomes forms) and to edit the patient’s profile. Many of these options are also available in the “view” button on the AT Dashboard.
The baseline button provides you the option to conduct a baseline SCAT5 or other functional ability baseline tests that includes:
Single leg balance
Weight-bearing lunge
Single leg hop
Side hop
Leg anterior reach
Square hop test
The quick info button provides a quick way to see all of the patients' demographic information, including:
Name
Graduation year
Date of Birth
Known medical conditions
Primary Guardian information
Most recent scores on outcome instruments and concussion testing.
The Quick Info button provides an easy way for the AT to identify important information about an athlete without opening completed forms.
The Forms section of the Patient Dashboard allows the AT to filter forms based on injuries. Using the drop-down arrow, select a specific injury for the patient. Once this is done, only forms associated with the selected injury are displayed in the forms list. If no filter is selected, all patient forms are displayed, with the default sorting by date completed.
Once an injury is selected, you can access the buttons that add new forms for the patient’s injury. These include adding an injury evaluation, daily treatment, discharge summary, and uploading injury files. In patients who sustained a concussion, adding a SCAT5 and Graded Symptom Checklist forms is also available through the concussion drop-down.
Optional patient reported outcome measures such as the AFAQ, GAD-7, TSK-11, and PHQ-9 are available under the psychological PRO's drop-down.
All of the patient’s forms are listed below the filter box. You can view forms or edit those forms that have not yet been completed and submitted. The same options as for the AT Dashboard exist. You can search by entering the form name, or sort by form status. You can also choose to display AT completed forms, All forms, or Patient completed forms.
