Check & Connect® is an evidence-based mentoring intervention focused on supporting school engagement. In Check & Connect®, a trained mentor tracks student progress in specific areas that are important to them moving toward graduation (that’s the “Check” part!). Then, mentors work directly with students to provide one-on-one support and problem-solve with students to increase student engagement at school and with learning (that’s the “Connect” part!). Mentors also help strengthen family-school partnerships to set students up for success. Learn more here.
The SDLMI is an evidence-based transition intervention with demonstrated in-school and postschool outcomes. It allows for personalization of goals and supports weekly activities where students learn a structured process to set and go after goals for postschool success. Find out more here.
History of Check & Connect® implementation
Capacity to have:
4 - 14 students who are on IEPs (Grade 9-12)
4 - 7 Check & Connect mentors (training for new mentors will be provided)
1 school or district-level coordinator (training for new coordinators will be provided)
Weekly 60-minute Check & Connect® + SDLMI sessions with Check & Connect® students
Improve students' transition and postsecondary outcomes
Support with data, training, implementation, and professional development
Access to new data assessments to improve program outcomes
$250 bonus each year for mentors and coordinators
Project Timeline
2024-2025, Year 1: Planning; recruit & train Cohort 1 school partners
2025-2027, Year 2-3: Cohort 1 schools implement with project team support;
Recruit & train Cohort 2 school partners
2027-2029, Year 4-5: Cohort 2 schools implement with project support
