The Complete Health & Wellness Center’s Nurse Practitioner, Physician Assistant, Registered Nurse and Medical Assistants are all employees of Premise Health. Premise Health partners with supervising physicians who provide oversight to the providers, as required by state law. These physicians conduct monthly chart reviews and are available for consultation on more complex cases as needed.

Nurse Pracitioner

Shelly Lynch, APRN, FNP-C

Shelly Lynch is from Kansas City, Kansas and currently lives hear Lee's Summit. She completed her education at the University of Missouri - Kansas City, where she earned a Masters in Nursing. Before joining the Complete Health & Wellness Center care team, Shelly worked as a Nurse Practitioner with Family Practice of West Central Missouri. She strives to treat the whole person and not just the disease state.

In her spare time, Shelly enjoys being outside, gardening, walking, experimenting with new recipes, reading, decorating, and mostly spending time with her husband and children.

Physician Assistant

Claire Exum, PA-C

Claire Exum is from Olathe, Kansas, and currently lives in Lenexa. She attended Washburn University for her undergraduate education and received her physician assistant training at the University of Oklahoma School of Community Medicine in Tulsa, Oklahoma.

During her training, Claire developed a love for primary care because it allowed her to build meaningful connections with her patients as she invested in their care. She practiced in primary care in Eugene, Oregon, before returning to the Kansas City area to be closer to family. Claire hopes to build a partnership with her patients to strive for optimal health in all aspects of their lives.

Claire loves to spend time with her husband, Eric, and their cat, Summer. She is an outdoor enthusiast who enjoys hiking, snowboarding, and stand-up paddleboard. In her free time, Claire also enjoys baking, hanging out at coffee shops, and going to concerts.

Physical Therapist

Jared Smiddy, PT, MPT

Jared Smiddy is an Illinois native who moved to Lee's Summit from Fort Collins, Colorado in 2021. Jared has extensive experience in outpatient orthopedics and is certified in dry needling. He is excited and honored to be able to serve LSR7 employees that are serving his own family.

He completed his undergraduate and graduate degrees at Maryville University of Saint Louis, where he also played basketball and met his amazing wife, Jen, who is also a physical therapist. Together, they have been blessed with two children, Jacob and Juliette, along with a Goldendoodle named Charlie.

In his free time, Jared enjoys sports, music, the great outdoors, watching or coaching his kids' sports, and making memories with his family.

Medical Assistant

Breonna Moore, MA

Bree Moore is a patient-focused, compassionate Nationally Registered Certified Medical Assistant with clinic experience since 2019. Her career has taken her to clinics across several states, where she has supported care teams and enhanced patient experiences. Bree is excited to bring her expertise to the patients of the Complete Health & Wellness Center. In her free time, she enjoys traveling and spending time with her children.

Medical Assistant

Diana Perez, MA

Diana Perez is a Medical Assistant from Lees Summit. Fresh out of high school Diana attended and graduated from Concorde Career College back in 2019. Previously Diana was a Medical Assistant at Family Health Care, a FQHC that served medically underserved areas and communities. She has experience in family practice, urgent care, OB, orthopedics, and infectious disease. Diana takes pride in being bilingual, speaking both Spanish and English. Her favorite part about working in healthcare is building trust and relationships with her patients and being able to educate patients on the importance of their health. In her free time she enjoys kayaking, fishing, and spending time with family.
