Our Services
Hospice care is a set of services provided in the home, free-standing hospice facilities, nursing homes, and other long-term care facilities to patients with a life-limiting illness. Hospice care focuses on caring, not curing. We provide support and assistance that allows the patient and family to live with dignity and give patient and caregivers the opportunity to participate in decisions regarding care.
We provide items to you that are related to your hospice diagnosis and what's needed to provide care.
Medication - We cover all medications that are directly related to your hospice diagnosis.
Medical Equipment - We provide items such as wheelchairs, beds, bedside commodes, etc. that will meet each individual's needs.
Supplies - We supplement supplies that are used in the home that are necessary for patient care.
Common Hospice Diagnoses
Meet Your Care Team
Complete Choice Hospice is dedicated to providing a wholistic approach to care for the patient and family. Your care team is made up of professional and experienced individuals who create and execute your care plan. Your team includes:
Medical Director
The medical director is a Georgia licensed physician responsible for overseeing the patient’s hospice care. The medical director reviews each patient’s plan of care to make sure all needs are being met in the best possible way.
Registered Nurse
All hospice nursing services are based on patient need and performed under a plan of care for that patient. All Complete Choice Hospice nurses are licensed professionals specifically trained to do hospice nursing.
Certified Nurse Aide
A certified nursing assistant (CNA) can visit on a regular schedule if you choose and is approved by the doctor. The CNA can provide personal hygiene care, such as bathing, washing hair, brushing teeth, giving back rubs, providing skin care, and changing bed linens as well as light housekeeping tasks.
Chaplain
Spiritual care needs are assessed by our chaplain, who will visit, or coordinate visits with other clergy, according to the wishes of the patient and family.
Social Worker
Our social workers offer emotional support, counseling, and coordination of community resources and support services, as well as helping with final arrangements.
Volunteer
Our trained volunteers do many things for patients and their families such as, light housework, cooking, running errands, and providing general companionship.
