Founded in 1979, Community Hospice & Palliative Care has grown into a pioneering national provider of hospice and palliative services, offering a wide range of programs designed to support the varied needs of the communities it serves.
Over the past 45 years, the organization has cared for nearly 220,000 adults and children across Baker, Clay, Duval, Nassau, and St. Johns counties, and has recently broadened its reach to 11 counties in north-central Florida. This expansion enables the team to support patients in Gainesville, Lake City, Palatka, and nearby areas.
With more than 950 staff members and 1,000 volunteers, Community Hospice & Palliative Care assists around 1,500 patients each day, providing comfort and support wherever they reside—whether at home, in long-term care or assisted living communities, in hospitals, or at one of its 10 inpatient care centers. The organization remains dedicated to enhancing the well-being of patients and their families, serving as a compassionate resource for end-of-life care across the region. Care is always provided, regardless of a person’s ability to pay.
