Our mission: To offer a wide variety of housing options to meet the needs of those individuals in Darke, Miami & Shelby Counties struggling with mental illness and/or substance abuse disorders, allowing them to work towards stable and permanent housing while in a safe environment.
Housing is Part of the Recovery Process
Community Housing can facilitate access to safe, affordable housing options for adults with mental illness and/or addictions. Referrals for services must be made by a Mental Health or Addictions Case Manager. Services are available for eligible individuals from Darke, Miami or Shelby Counties.

Providing in-reach programming and services for people with mental health and substance abuse issues currently incarcerated in Darke, Miami and Shelby County jails.
About the Re-Entry and Recovery Program
Our program provides in-reach – we visit people in the local jail—programming and services for people with mental health or addiction issues who are currently incarcerated in the Darke, Miami or Shelby County jails.
We assist them by facilitating and coordinating care as people transition from the county jail into their home community. We can also assist those who are in prison and from Darke, Miami or Shelby Counties.

Face to face assessment while in the jail
Referral to Recovery Housing (Level III certified)
Referral to Residential Treatment upon release

Referral to Mental Health and/or Substance Abuse Treatment
Intake appointments
Referral to the Vivitrol® medication-assisted treatment program, to start while in jail
Benefits—Medicaid, Food Stamps, etc.
Help with obtaining ID, birth certificate, Social Security Card, driver’s license
