For Campo students:
Click the button below to access the Dance Agreement & Permission Slip forms
Fill out electronic form and submit (no need to submit a paper form to the office)
Forms are due no later than Wednesday, April 15th
If your guest is NOT a Campo student:
Your guest will need to complete a paper version of the "Dance Guest Agreement Form", which includes obtaining the signature and business card of an admin at their school.
Please note this form is not electronic; a hard copy will need to be turned in to the office.
Plan accordingly to meet the deadline as this can take a few days.
