Report of Personnel Change (.pdf) - Submit this form after nominating a candidate who meets the established minimum qualifications for a position. Use also to track changes in title, position status and salary for each employee from original appointment through separation from service.

This form is used by civil service job candidates who were bypassed for original appointment or promotion. The appeal form must be filed with the Commission within sixty calendar days after the job candidates receives the reasons for bypass.

The Civil Service Commission hears appeals from City employees regarding their employment. If you feel wrongful action has been taken against you at work, you might have grounds for an appeal. You can use these forms to file an appeal.

Performance reviews for Faculty, Academic Professional, and Open Range Civil Service staff are a factor in determining employee compensation under University policies related to merit-based salary programs.

Civil Service appeals will now be accepted by email, US mail or in person. To file an appeal, visit our Appeal and Subpoenas forms page to download an appeal form. Once you have completed the appeals form, you may mail or bring it to the Civil Service office at the address on the form or you may email it to csappeals@nola.gov.

Applications for Employment in Civil Service positions with the City of New Orleans are taken for specific positions for which we anticipate vacancies. Attach your Certificate of Release or Discharge from Active Duty, DD 214 (or equivalent), and any available records of pay or promotions to the RI 20-97 and send to the appropriate military finance center. The guides provide a cursory view of format and content to help applicants prepare for the indicated exam. Please refer to the NYSCSC's Web site above to access the most current version of test guides offered.

The worker mails the original to the U. S. Office of Personnel Management (the address is preprinted on the form) and keeps the duplicate in the case record. Attach Form H0003, Authorization to Furnish Information (signed by the applicant, recipient, or responsible person), to the original.

After completing the civil service exam, applicants should await notification from the Candidate Assessment Division to open an account and register as an NYPD civilian job applicant. Follow the prompts at this site and complete the CAS-5A Personal History Questionnaire online; then submit it online. DO NOT MAIL the questionnaire.

The NYPD's Candidate Assessment Division (CAD) will only accept civilian applicant forms from candidates who have successfully completed the written civil service examination.
