2023 CIT Information
I need to receive all completed Verification of Volunteer Hours Forms and MHS 2023-2024 Community Service Hours Forms TOGETHER in one email from each CIT. Fill them out completely making sure you total your hours by day and month and include your grand total hours for the entire summer on the Verification of Volunteer Hours Forms. Write the same grand total on the MHS 2023-2024 Community Service Hours Form.
What if I don't have my Verification of Volunteer Hours Form?
- If you already turned in a hard copy of your Verification of Volunteer Hours Form (without the MHS 2023-2024 Community Service Hours Form) or left it with your Site Supervisor or at the camp you supported, you should find a scanned copy of it HERE. Please follow the steps listed below. If you do not see a copy of your scanned form and you don't have the form, you will need to complete and submit a new form.
1) Print it out. 2) Make sure you have totaled your hours by day and month and entered your grand total hours. 3) Print out the MHS 2023-2024 Community Service Form and complete it. 4) Make sure that the total hours you list on the Verification of Volunteer Hours Form match the total hours you list on the MHS 2023-2024 Community Service Form.5) Scan (or take clean pictures) of both forms and email them to me at RCorbin@moorestown.nj.us.
I will review the forms, sign them, and scan/email them back to you so that you can turn them in to the high school.
Note: All volunteer hours need to be captured on the Verification of Volunteer Hours Form, not on a time card. If your hours are currently on a time card, please transfer them to the Verification of Volunteer Hours Form.
