Option A: Attend the annual Sport Physicals held by Channel Islands High School 

Athletics on Thursday, June 22nd, 2023 at 2pm. Cost is $25. 

OR

Option B: Contact family doctor or visit an Urgent Care facility to receive a "sports

physical" or "athletic physical" in order to be cleared to do physical activity and

participate in practices. Please refer to the physical as either sports or athletic, so

that the doctor knows they are doing the physical to clear you for sports and not

doing a full physical. THEY MUST USE THE FORM INCLUDED IN THE PACKET,     

TITLED: Oxnard Union High School District PREPARTICIPATION PHYSICAL 

EXAMINATION FORM. DO NOT forget that paperwork at home!!

Upload the forms (take pictures or use an app to scan them to pdf files) to homecampus.com, then turn in hard copy to Coach Raq.
