Following the steps and procedures listed below help ensure students' records are submitted by the colleges' application deadlines. Students should follow CHS Transcript Request deadlines to ensure records are sent on time.

A Transcript Release Form must be on file with the Transcript Office in order for CHS to release official transcripts to the colleges and scholarship organizations you request.
Many seniors already submitted their Transcript Release forms in the Spring. This form only needs to be submitted once.

Be sure you have included:
Name (Last, First), Student ID #, Preferred Email Address & Counselor Name
Application Deadline Date, Admission Type (Regular, Early Decision, Early Action, etc.), Application Method, Name of College, City and State/County of College
Student and Parent/Guardian Signatures (electronic signatures are okay!)
Select 'School Store' at the top of the page, then 'Browse All Items'.
Find the option for "Official Transcript Request Fee'.
