Parent/Guardian Release Agreement:
I/we affirm that the named child is physically able to join in all Chicopee Blaze sports activities and has no physical or mental impairments. I, both individually and as a parent/guardian, give consent for my child to participate in any and all Chicopee Blaze activities. I acknowledge and accept all risks and hazards associated with such participation, including transportation to and from the activities.
I hereby waive and release Chicopee Blaze, its Coaches, Organizers, Sponsors, Supervision Staff, Participants, and any other affiliates, along with persons transporting my child to and from activities, from any liability for bodily injury, personal injury, or property damage arising from my child's participation in any and all activities.
Photographs and media:
I grant The Chicopee Blaze Team the non-retractable right and permission, within our local community, to use photographs and video of myself or my child, taken by The Chicopee Blaze Team or provided by me to The Chicopee Blaze Team, for various purposes including promotional, advertising, and trade uses.
Behavior Expectations/Code of Conduct:
We kindly ask all parents, players, and families involved with the Chicopee Blaze Team to remember that everyone represents our team. The success of every aspect of our program relies on the invaluable contributions of many individuals.
We strictly enforce all policy rules regarding profanity, verbal conduct, smoking, alcoholic beverages, and physical threats. We reserve the right to remove any player from our team due to poor conduct, with no refunds issued in such cases. Any inappropriate behavior towards coaches, teammates, or fellow parents will not be tolerated.
Players are expected to show up to games and practices on time. Players must make every effort to attend all practices and games and will notify a coach if any circumstances arise. Players who do not attend practices and skip games without notice may not get as much play time due to missing important information such as plays that will be run during games.
If transportation is an issue, please notify coaches, and we can arrange a ride with notice at least a day ahead. If you anticipate transportation being an issue throughout the year, please let us know, and we will provide assistance.
Let's remember that we all play a crucial role in the success of our basketball team. By working together, we can achieve great things both on and off the court.
