Acquire a sports physical (blank copy provided below) and BRING THE COMPLETED FORM TO IN-PERSON TRYOUTS.
Fill out the waiver provided below and BRING THE COMPLETED FORM TO IN-PERSON TRYOUTS.
*if you do not have your sports physical & waiver turned in at the beginning of in-person tryouts you will not be permitted to participate*
