Failure to complete this form will delay the activation of your recreation area access card.  Pool card will not be issued or activated if this Resident Information form is not submitted.

Residents and renters must be in good standing with Charleston HOA covenants/bylaws.  If household has a violation notice with Charleston HOA Beautification Committee, this will delay gate card activation and/or temporarily suspend until violation has been fixed.
