Printed waivers will be available at the Front Desk!

Waiver Instructions

1. Either print or pick-up a waiver form.

2. Fill out the form with: Student Name, Activity Name, Date and Parent Signature.

3. Return completed waiver to the front desk or email directly to whomever is the contact for the event you would like to attend.
