Classroom Support Program Reimbursement Request

 Classroom teachers may this form to request reimbursements of up to $350 total annually for classroom-related purchases (instructional materials, art supplies, books & educational games, organization tools, or classroom décor) Subscriptions and computer hardware are not eligible. Computer hardware needs to be submitted as a Teacher Grant.

PTA Teacher Grant Application

 PTA Members may use this form to apply for one-time grants that support classroom or school initiatives not available using NMUSD site funds. Application must be approved by the department Chair before submitting.

General PTA Reimbursement Request

 This form can be used to request reimbursement for any PTA-approved expenses including approved PTA Grant expenses.
