Class III antiarrhythmic
Action:
Acts directly on the myocardium to delay repolarization and increase the duration of the action potential. Amiodarone also blocks the potassium channels , preventing cardiac arrhythmias.
Indications:
Ventricular arrhythmias, second-line treatment for atrial arrhythmias
Contraindications:
Sick sinus syndrome, second and third degree heart blocks, cardiogenic shock, when episodes of bradycardia have caused syncope, sensitivity to benzyl alcohol and iodine
Dose/Route:
Ventricular Fibrillation and Pulseless Ventricular Tachycardia
Adults: 300 mg IV/IO, may be followed by one dose of 150 mg in 3-5 minutes
Children: 5 mg/kg IV/IO (max dose of 300 mg), may repeat 5 mg/kg up to 15 mg/kg
Monomorphic Tachycardia and Narrow Complex Tachycardia
Adults: 150 mg in 100 ml of D5W, infuse over 10 minutes IV/IO
Post-Conversion Maintenance Drip
Adult: 150 mg in 100 ml of D5W, administer at a rate of 40 ml/hr IV/IO
