• New students should complete your enrollment packet and return to Mrs. Karup in the front office.

For any medication to be taken at school (or on school trips outside of school hours), click here for the Authorization for Medication Form.

• Student Device Insurance Coverage & Agreement (Optional, but recommended) 

This form should be completed if your child plans to ride the bus. A new form must be completed even if the student rode the bus last school year.
