The patient's name will appear in the title of the individual page, followed by the reason for visit. Below this information, patient weight, height, blood type, and blood pressure will be listed. Any X-rays, MRI or CT scans taken/ordered are required as a portion of each individual chart. The next required section included should be a detailed explanation of how injury occurred. Remind patient that any background information withheld only hinders the treatment and healing process. If patient is unconscious or unable to speak on behalf of themselves, a patient advocate may provide any information post request of legal documentation. Any and all treatment including but not limited to injections, wound care, surgery, etc. are required to be recorded in patient's chart.

In the case of death prior to ability to treat, transportation service is responsible for all documentation. Otherwise, documentation including death certification regarding cause and time must be recorded within 24 hours by each individual administering treatment. If death occurs do to homicide or possible foul-play, legal action should be taken only by the county in which a lawyer will require copies of all medical documentation and records.

If patient expresses any indication of hallucinations, seeing/hearing things or people that do not exist, or paranoia they must be monitored and/or a psychological evaluation must be performed.
