How to setup the add-on?
First, choose Add-ons --> Form to Calendar and select Connect to Calendar.
Then, a sidebar named Connect to Calendar appears, click the Connect button. After that, click the View Result button to submit data to create an event.
The input date is an event date.
Note: You change change the text/question such as "Event title", "Event start date", etc... just NOT change the type of items (date/time).

Try one of these solutions:

If you have multiple accounts logged in, sign/log out others account except the one you need to edit the form.
