Instructions:

• Use this form to submit both your funding worksheet and your PD Annual Plan (or the combined document above).

• You can either:

  •  Fill them out digitally and submit them (Preferred)

  • Or, scan a print copy, save it as a PDF*, then submit that PDF file (please make sure it is legible)

*.jpg or other image types will not be accepted
