REGISTRATION

• Owners must provide medical record proof that the vaccinations of cats are up to date

• All cats must be transported to C-A-T Playground in a recommended pet carry box

• Cats must be free of fleas. If at any time your cat is found to have fleas and ticks, we may provide the appropriate flea or tick removal treatment, and you authorize us to provide such service at your additional expense

• No cat suffering from or suspected to be suffering from any infection or contagious disease can be accepted

• Any cats under medication or sickness will not be accepted

FOODS AND LITTERS

• Owners need provide own foods and litters for cats. C-A-T Playground does provide foods and litters in case shortage of foods given occurs with additional charge of RM5/day.

CHECKING OUT TIME

• Cats check-out time is before 1.00 P.M. If exceeds, then additional charge may be inflicted for every extra hour late.

• Owners must be highly responsible in picking up and collecting their cats on time.

• If the cats are not collected after 72 hours of check-out time and date, C-A-T Playground reserves the rights to re-home the cats.

• Owners cannot simply abandon their cats at the facility

• If you plan for someone else to pick up your pet, we must be informed min 24 hrs in advance by the owner providing all necessary details eg, name, IC. contact etc for verification purposes

• The management of C-A-T Playground is then released of their responsibilities for the pets boarded thereafter under these terms

CHARGES AND PAYMENT

• All charges for boarding are to be made full in advance and other services that are due on or before the pet is released

• Payments are not refundable. Once service is affected, no further claims shall be entertained

• Monthly bookings must be pre-paid for that entire month regardless if the animal leaves the property for short or extended periods of time. This is to ensure that your placement for your pet will be held and not be rebooked for another client, should you need to bring your pet back for a short leave of absence from the facility, otherwise you run the risk of losing your placement

EMERGENCY MEDICAL CARE

• Upon endorsing the boarding form, you will also be consenting to emergency medical care in the event your pet becomes seriously ill. If you leave us an emergency phone number, we will certainly try to reach you and inform you of the situation. However, you can be sure the veterinarians will provide the best care possible for your pet's condition. Such charges shall be borne by the owner

• The management reserves the sole right to do as they deem appropriate thereafter without further notice to the owner

• You acknowledge and agree that in the unlikely event your Cat becomes ill or injured, or if your Cat has a pre-existing condition which is aggravated by its stay, and requires professional attention we will attempt to notify you at the telephone numbers you provide on the reverse side. If we cannot reach you, C-A-T Playground at its sole discretion, may engage the services of a veterinarian and/or administer medicine or give other necessary attention to your Cat, and you authorize us to provide any such service at your additional expense. In cases we believe to be critical, we may take your Cat to the veterinarian first before trying to contact. If you refuse medical treatment for your Cat, C-A-T Playground, at its sole discretion, may engage the services of a veterinarian and/or administer medicine to make your Cat as comfortable as possible until picked up by you

• All clients shall be in agreement to these terms once they have chosen at their freewill to sign this form and board their pets at C-A-T Playground

• If the cats suffered any injuries, sickness, accidents, missing or death during their stay, C-A-T Playground will not be accountable for them
