Sign a Waiver

For Participants ages 17 and under. A Parent / Legal Guardian, over the age of 18, must fill out this waiver for participant prior to participation.

For Participants ages 18 and up. Participant must sign a waiver prior to participating in any event with Bushido Parkour Ninja Fitness.

For Multiple Participants of varying age. If under the age of 18, a Parent / Legal Guardian must sign the waiver.
