To maintain social distancing we ask that people with last names starting with:

1. A and B come at 16:00 (4 pm)

2. C to G at 16:15 (4:15 pm)

3. H to L at 16:30 (4:30 pm)

4. M and N at 16:45 (4:45 pm)

5. O to Z at 17:00 (5 pm)

Please bring your passport (or a photo of your passport on your mobile) and make sure to wear a mask and maintain at least 2-meter social distance

Once at the site, most likely you'll need to collect and accurately fill-out/sign paper copies of:

1. Your Ham Tien government-issued vaccination invitation

2. A vaccination consent form (sample form provided for your reference under FAQs)

3. A health declaration form (sample form provided for your reference under FAQs)

Please note: on-site medical staff at the vaccination location will fill-out the following 2 parts towards the end of the health declaration form

A: SÀNG LỌC/SCREENING

On-site medical staff will will fill-out questions 9 and 10 (last 2 questions of this section)

B: KẾT LUẬN/CONCLUSION/ЗАКЛЮЧЕНИЕ

On-site medical staff will will fill-out this entire section, and date/time/sign at the end of this section/form

We expect to staff with English and Russian-speaking volunteers

These 3 documents will be required for Step 3

You will also be provided an appointment time and location for Step 3

We expect you will need to visit the sport stadium in Phan Thiet (about 1km from Lotte Mart), most likely at 14:00 ( 2 pm):

Please bring the 3 documents from Step 2 and your passport (or a photo of your passport on your mobile). Please make sure you complete each line item/question in the documents provided in Step 2, prior to arriving at the vaccination site.

Make sure to wear a mask and maintain at least 2-meter social distance

Once at the site:

1. You will need to provide the 3 completed documents from Step 2

As noted under Step 2 above, on-site medical staff at the vaccination location will fill-out the 2 parts towards the end of the health declaration form, and should therefore be left blank

2. You will receive your vaccination shot

3. It is advised you wait at least 30 minutes post receiving your vaccination shot, to make sure that your body does not have any adverse health reactions to the vaccine

4. You will receive your vaccination confirmation card

5. Please make sure that your vaccination confirmation card has all of your information (e.g., name, passport number, etc.) that is accurate. If not accurate, there will be a desk you can ask for that will make any necessary corrections.
