Requirements:

• Include a picture (either taken by you at the facility WITH PERMISSION, or found on the internet) to help describe the experience. 

• Provide a clear explanation of the experience.

• Include WHY you found this experience to be so amazing.

Things to keep in mind:

• Due to HIPAA, please DO NOT use patient's real names. 

• DO NOT INCLUDE PATIENT FACES IN YOUR PHOTOS (again, HIPAA), and always ask permission from your supervisor and the patient to include your photo in the portfolio.

• Everyone defines "amazing" differently, and the reasons something feels "amazing" will vary from person to person. This is your opportunity to share how your practicum experience has made an impact on your life - your story, in your own words.
