Fill Out Student Permission Slip (Each Performer Must Complete)
Submit Act Video ( MP4), Music File ( MP3) and 2 Act Photos (One Email Per Act) - send to bradleyhillsvarietyshow@gmail.com
Make sure you name all the files according the name of the act.
