You can get a consent form at the front office of your child's school. After it has been filled out completely and signed in ink, you may return it to your child's school and they will forward it to the clinic for enrollment. Only original, signed consent forms are legally acceptable.
The clinic is for Beverly school-aged residents in grades K-12.
It is not necessary to schedule an appointment. The clinic makes arrangements with your child's school to transport them to the clinic by bus on a scheduled day. They will be returned to school when the appointment is finished.
You will receive written confirmation of your child's appointment with details about what treatment they received while at the clinic. It is required that all patients see a dentist for examination within 90 days of a dental visit with the Public Health Hygienist. For those who do not have a regular dentist, a list of local dentists will be provided. It is the responsibility of the parent/guardian to coordinate and pay for the dental examination.

The clinic is located in the Health Center at Beverly High School (100 Sohier Rd).
MassHealth will cover the cost of the dental work performed at the Beverly Dental Clinic. There is no fee for those children who do not have MassHealth coverage.
