-Hello/Good morning/Good afternoon this is ______________ I am calling from the insurance center for _______________ (lender name) on a recorded line, and I would like to (verify coverages / confirm a cancellation received / request a declaration page etc.)

Is policy active /cancelled? Current effective Dates?
If a cancellation is currently in system, is or was there a lapse in coverage? And if so, what are the lapse dates?
Is lienholder listed correctly with insurance tracking center address? (look at lender rules for correct address depending on what lender you are working on, also verify correct spelling on lienholder name)
Is there physical damage deductibles? (Comprehensive and collision; and if they meet the lender requirement?)
Is borrower/co-borrower in TC listed as named insured/s on policy? (insured /covered drivers, NOT excluded)
(WHEN APPLICABLE – for lenders that specifically require. CARLISLE MOTORS for example) Verify policy type… “Is this a limited/ named driver only policy” or “is it a standard permissive use policy”. For these lenders in particular, also verify if policy carries ANY excluded drivers.
Request a full declaration page showing all above requirements.
If agent is faxing or emailing proof of insurance showing a driver you verified please make sure an insurance card (if the declaration page does not show driver) is being sent.
If policy is active and meets lender requirements, PLEASE PEND and document in notes what was spoken.
Notes SHOULD be very detailed…Name of agent/rep you spoke to and all info mentioned above!
If sending a fax / email request to the agency, please keep and follow up on the request to make sure agency received it and is working on getting policy to the insurance center.
