Your doctor's office may already have these but you can also print and bring this one. Athletes must have a current physical (dated after April 15, 2023) in order to participate in practice beginning August 7.
This is a fillable PDF. Fill out a separate form for each athlete and email to: jonesc@benzieschools.net
Please print and fill one of these out prior to the season. We will need drivers for camp and on occasion some meets depending upon the bus situation. It just makes it easier if this is already on file with the athletic office. You will need a copy of your driver's license as well. It can be turned in to Mr. Harris or one of the Kelly's.
