Type of Incident
No Harm: Any process or variation that has occurred but did not affect the outcome or cause harm.
Near Miss: Any process variation which did not affect the outcome but for which a recurrence carries a significant chance of a serious adverse outcome.
Adverse: Adverse events are unexpected incidents, therapeutic misadventures, iatrogenic injuries, or other adverse occurrences directly associated with care or services provided.
Sentinel: An unexpected occurrence involving death or serious physical or psychosocial injury, or the risk thereof.
Unknown: An incident where the cause or circumstances cannot be definitively determined.
