Steps to receiving your Permit

Once you have taken and passed the permit test here at Bay Area Driving School you will need to go to the D.P.S. office to complete your eye exam and take your picture the following is a list of things you need to bring with you in order to do so

Verification of Enrollment (VOE)

Go to your front office at your High School and ask them for a VOE you will fill out the sheet and you should receive it by the following business day.

2. Birth Certificate (official or certified copy)

3. Social Security Card (official or certified copy)

4. Application

You will receive this from your classroom instructor. He/She will explain how to fill it out. DO NOT SIGN IT until you are in front of the D.P.S. clerk.

5. DE-964 Form

You will receive this form from the front office at Bay Area Driving School after you have completed and passed your permit test.

6. $16.00 Fee

7. School ID or Report Card

8. Two proofs of residency
