We're heading into the second quarter of the new year...my, how the days are flying by. And where do I begin? I know every one of you is busy making this hospital and community a healthier and safer place. The most recent Advanced Leadership Seminar participants are getting out into the community and teaching life skills and even within the hospital, staff are getting more involved to support a more just work culture.
Despite being busy, which seems to be our status quo at GMHA, I want to take some time to address the recent Employee Survey. I have personally read both the compiled aggregate data and all the comments expressed by employees, who responded to the survey. I am not shy or blind to the inherent issues we face at our hospital. Some of our limitations can be related to being a government entity, with more restrictions than a private facility, which can cause hurdles and delays in decisions. Despite our limitations, I assure all of you that I hear you and I respect your feedback. The Senior Leadership team and I are working as best, and as responsively, as we can to address your concerns. I'll admit, some are more easily fixable than others; some issues require more investigation, training and research. Nonetheless, we're committed to your well-being, happiness and the overall longevity of our GMHA team. Thank you to those who responded. March was Social Worker Month and March 15 was World Social Work Day. The Governor signed a proclamation for Social Worker Month on Friday March 4th. Shout out to our amazing GMHA Social Work team!

Patient Safety Awareness Week was March 13-19. The Compliance Office put together a rocking PSAW Fair! March 31st was the deadline to complete the PSAW online mandatory training. If you have not completed it, please contact the Compliance Office or our Patient Safety Officer, Eric Lee, as soon as possible.
March 13-19 was also AmeriCorps week...thank you to our INCREDIBLE AmeriCorps volunteers at GMHA!
March 30 was National Doctors Day. Thank you to the wonderful, compassionate doctors of GMHA!
LET'S GET DOWN TO BUSINESS:
We held a donation ceremony for JAG Technology, a Taiwanese telehealth company, which donated outpatient monitoring software to GMHA. The intention is to work with Public Health and other community partners to fully utilize this technology to better preventative and proactive care.
The EHR build is back!! The 4th floor classroom is once again set up for all of our amazing informatics team members and SMEs to get back to building. With an aggressive timeline and everyone back on track for building and integrated testing...the current GO-LIVE date (barring any unforeseen surges or circumstances) is Sunday August 7, 2022!
ACCREDITATION - we're still moving forward in our pursuit of accreditation with the Center for Improvement in Healthcare Quality (CIHQ). The Compliance Office is ramping up training and courses that all GMHA staff must take as we prepare for accreditation. MAJOR UPDATE: we are tracking for a Life-Safety Mock Survey set for May 2-4, 2022. This will be followed by a full Mock Survey with a nurse and physician later this year, likely in October or November. Stand by for more details as we get closer! Exciting stuff!
As a reminder: CMS is mandating COVID screening for all staff, visitors, contractors, vendors, volunteers, students (etc) of GMHA, before they enter the building. A digital screening is now available online. You may complete the questionnaire one hour before you arrive to work: tinyurl.com/gmhscreen. Please note that we have not fully transitioned to a digital-only screening, and that you will continue to answer questions at the door and receive a sticker until further notice. Thank you for your cooperation.
PLEASE REMEMBER: During a CMS survey, you may be asked what the GMHA COVID screening process is. Whether you answer online or in-person, the process is still: answering 3 mandatory screening questions and based on the answers, receiving a sticker, which indicates that you are cleared and good to go in and out of the building for that day.
While our COVID numbers are down, both in the community and at the hospital, we are still on-guard and ready to respond to any surges or increases in cases/patients. We are all so burnt out and just recently, some members of our Senior Leadership were at the Guam Legislature testifying against a resolution to end the Public Health Emergency. We're trying to fight for what's best for our community.
Are you wearing an approved mask? All staff working in patient care areas must be wearing a KN95 mask (equivalent or higher) at all times. A NIOSH approved N95 respirator must be used in all COVID-patient and/or airborne isolation rooms. GMHA will provide all staff in clinical areas a new, approved KN95 (equivalent or higher) during every shift. PLEASE NOTE: if your department requires KN95 masks (your staff works in clinical/patient-care areas), they can be ordered on your regular weekly order. It's stock #111641 and they come 50 per pack. New Hospital Update: the preferred land site for the new Medical Campus (Mangilao/Eagle's Field) is still being evaluated by the Federal Government for alternate use. This is not stopping GovGuam/the Governor's pursuit of a valid location for our new hospital. The inter-agency conversations are continuing, as we re-assess all available land and try to work on finalizing the best, most central and easily accessible area for our island community.
Capital Improvement Projects are going strong at GMHA:
The Z-Wing demolition bid has been awarded and the work is expected to begin as soon as permitting is complete. This first phase should be complete by early summer. This is a 3 phase project and during the last phase, the hospital is expected 20-30 parking spots in the footprint of the demolished Z-Wing area.
Work has begun on the roof and envelope upgrade project at our hospital. This too is in the permitting phase and we look forward to the project beginning shortly, to enhance the structural integrity of our facility during heavy rainfall, mitigating against potential leaks, and sustaining the building for the next 5-7 years until a move to our new hospital.

We are still strongly encouraging ALL GMHA staff to get their COVID-19 BOOSTER Shots. There are many employees currently due for a booster (6 months or longer since their second shot). You may call the Employee Health office at 671-648-6765 to make an appointment to receive the booster shot.
The Flu Shot clinic is still running from 8am-5pm, Monday-Friday on a walk-in basis in the Employee Health office.

LET'S GET DOWN TO BUSINESS:
We anticipate much movement on the CIP front this month! It's an exciting time at GMHA!
First, the removal and replacement of 14 Air Handling-Units (ACs) is scheduled to begin on April 4. This will help temperature control around the hospital.
The Z-Wing permits are expected this month for work to begin on the first phase of the demolition.
We're also expecting permitting to push forward for the roof and envelope upgrade. Please continue to track circulars and other pertinent information as information becomes available.
The latest group of GMHA employees to complete the Advanced Leadership Seminar is putting their new knowledge to work in a life-skills workshop. In collaboration with other GovGuam agencies and media partners, the 11 GMHA employees will be out on Saturday April 9 to teach island youth essential life skills. As we get closer to accreditation, the Compliance Office will be sending out more courses and tests to complete, in order to get us ready for the Mock Surveys and eventually, the true survey. Please stay tuned to upcoming communications from the Compliance Office and please read the Compliance Corner information below to help us get geared up!
As part of GMHA's pursuit of accreditation, our agency is currently undertaking a massive effort to digitize and update all of our internal policies. Led by Han Seo and each department's Subject Matter Expert (SME), this project utilizes the MCN Healthcare Policy Manager Software to upload all current GMHA policies online. MCN is a tool that will help GMHA automate the policy approval and tracking process, as well as streamlining policy management. Please get in contact with Han to update your Department's policies/processes: han.seo@gmha.org or 671-482-1509. NOTE: the soft launch of the MCN website will be on Monday April 11, 2022 and you should be receiving communication this week. By July 11, the policies on the GMHA website page will no longer live there and everyone will have to access the MCN website for policies. If you have any questions, please contact Han Seo (information above).
As part of complying with the HIPAA Privacy regulations, all hospital employees, medical staff and volunteers must take, and successfully complete, a 1-hour HIPAA Awareness Training. This training provides an overview of the HIPAA Privacy regulations as well as information and guidelines on each person's responsibilities under HIPAA to protect and keep PHI (Protected Health Information) private and confidential. The training is mandatory, self-paced and provided in an online, web based format which may be completed over multiple sessions. Once completed, certification is good for 1 year. Effective February 1st, 2022, GMHA's HIPAA training will be absorbed into the Medical Group Management Association (MGMA) education platform, which in turn will serve as the hospital’s centralized location for mandated trainings. You will receive notification if your training is expiring. If you have questions, please contact GMHA Risk Manager, Kyle Dalman at 671-648-1365.
You don't need to wait for assigned courses! MGMA is open and accessible to all our staff. Using your GMHA email, you may peruse and search for relevant courses to your job/department and invest in yourself! Be a part of our growth at GMHA and help us become an agency dedicated to lifelong learning.

Some big shout outs to Hospital Pharmacist - Jhoana Gonzales. In addition to long hours at GMH, she is president-elect of the Guam Pharmacy Association, did community outreach with the Guam Patient and Family Advisory Council on COVID 19 vaccines, and also does compliance in-services for the GMH pharmacy staff. Your team appreciates you!

The GMHA Medical Staff Office put together a wonderful breakfast and lunch for Doctors Day on Friday April 1. It was amazing to see so many GMHA physicians come through and the wonderful camaraderie amonst our teams. GREAT WORK MEDSTAFF!!! 👏🏼
We're showcasing our two wonderful Eligibility Specialist staff in the Patient Registration department this month, Cara Cruz and Morthiza Soto.
Cara has been with GMHA for almost 3 years, while Morthiza has been here for 5. Both started as Patient Services Representatives before transitioning to Eligibility Specialists.
What do the Eligibility Specialists do?
An Eligibility Specialist generates a list of Self Pay patients on a daily basis, conducts room-to-room visits to discuss insurance status with patients, and they conduct interviews to determine eligibility for MAP/MIP, SNAP and Cash Welfare. The job requires working hand-in-hand with Social Services, Case Managers and Utilization Review to ensure patient’s needs are met in a timely manner. Applications are reviewed and processed at DPHSS, and case numbers on approved accounts are forwarded to Patient Affairs for re-billing.
Though humble, these two ladies have helped countless patients. They remove the stress off patients when they're able to qualify them for subsidized healthcare or social support...and, they ensure GMHA gets paid by self-pay patients, when they qualify for government programs.
Cara says the best part of her job is the ability to help GMHA patients succeed with their Medicaid/MIP application process and get approved. "Most especially our most vulnerable patients, with no family willing or wanting to help or no family at all, the homeless, etc. Knowing that they can get the assistance they’re entitled to and reassuring them the relief of having to worry about a bill and the ability to obtain prescriptions upon their discharge."
As for Morthiza, she says, "knowing that I get to help patients obtain insurance to cover their hospital bills, and for them to afford their medicine and other equipment for healthcare, at the same time adding to the Hospital’s revenue, is a daily accomplishment for me which I am proud of."
Both these ladies are mamas, with Cara having two children in their young teens and Morthiza has a middle-school aged son. Both don't have pets although Morthiza's son is working really hard to get straight-A's in the hopes his mama will give in and get him a puppy! 🐶
Both Cara and Morthiza say the best part of being at GMHA is the people! Cara says, "I’m surrounded by so many amazing coworkers, each with their own style and personality. I’m exposed to many different challenges on the daily, but with a great team we manage to overcome barriers through collaborative and effective efforts. Like they say, ‘Teamwork makes the Dream work’. " And Morthiza shares that GMHA is awesome because everyone is willing to help in any way they can and are very accommodating!
When she's not at work, Cara's favorite thing to do is spend quality time with her family...and with such a demanding job, she also ensures she gets her rest in during off hours. Morthiza also loves spending time with her son and her family. She says she also loves cooking and eating...you can catch her foodie pics on insta: @matsaiii...while you're there, be sure to give GMHA a follow too @GMH_Guam 😉

Dr. Dennis Sarmiento is our Rock Star Doctor this month!
Dr. Sarmiento is a Pediatrician and has been working at GMHA since September 2002 - happy almost 20 years Doc! 🤩
The physician is originally from the Philippines but has been living on Guam since moving to GMHA in 2002. He says nearly half his life has been here on island!
Dr. Sarmiento graduated with his bachelors in Medical Technology from Far Eastern University in PI. He then went onto MedSchool at the same institution, graduating in 1991. He ventured over to New York in 1997, where he completed a Pediatric Residency at Mt. Sinai Services-NYU. After 3 years of residency, Dr. Sarmiento decided to continue his training in the subspecialty of Neonatology at the University of California. However, after one year of training, the Doc got an opportunity to work as a pediatrician on Guam and took it!
Dr. Sarmiento has been married to his MedSchool classmate for 29 years now! 🤩 They have one daughter, who is a singer and currently at the University of California, Davis. The Doc says on his off days, he plays golf to de-stress. He also loves spending time with his family and friends...and when he's lucky, he can bring family, friends and golf together as the ultimate relaxation! 🏅
He says his favorite part of working at GMHA is the great opportunity to work with excellent medical staff who are experienced, knowledgeable, and efficient within their own scope of work, "which makes my job as a provider much easier and I can do it with confidence." He also says that, "being part of the GMH family provides me with personal satisfaction, because I'm able to share my knowledge, ideas and provide medical care to sick children on our island."
As for his message to all GMHA staff: "I would like to take this opportunity to thank everyone, especially the administration, medical providers, nurses and all of the GMHA staff, for the commendation given to me. It has been an honor to work with remarkable competent people. I honestly appreciate all of the effort, sincerity and understanding the GMH family has afforded me throughout the 20 years of my service here. I will forever be grateful for being a part of this great team."

Brandon Sevella, RN was the nurse on duty for a telemetry patient, when he noticed the same name alerts for two patients in his unit. Because of his watchful eye, attention to detail and dedication to medication/patient safety, Nurse Brandon ensured the right medication got to the right patient.
Thank you, Brandon for being a patient safety champion and upholding the highest level of safety and quality practices for our patients. Thank you also for entering this event into the Safety Learning System.
The Good Catch Award was developed based on best practices from national patient safety standards and ECRI, our Patient Safety Organization. A good catch is defined as an event reported that might have caused an adverse event/outcome, but was prevented due to active recovery efforts by caregivers. The overall goal of giving recipients the bi-weekly Good Catch Award, is to improve GMHA’s heightened awareness of best practices in patient safety and to support a positive climate where patient safety is top priority. It also helps produce an organizational culture of safety, free from punitive blame.

In a hybrid in-person and online ceremony in our GMHA boardroom, Taiwan's JAG TeleCare donated a remote patient monitoring platform to GMHA, to help bolster our island's healthcare and outpatient monitoring capabilities. The intention is for GMHA to work with community clinics and Public Health to ensure full use of this software.

Hospital Accreditation
The Right to Titrate
Titration is increasing or decreasing a drug infusion to maintain therapeutic effect. What nurse wants to call the attending every 30 minutes to notify them that the medication is not having the desired effect? On the flip side, what provider wants to be notified every 30 minutes that the patient on Propofol is at a RASS of 0 instead of the desired -2? Managed correctly, a titratable infusion is a beautiful timesaver and safe way to manage a patient’s response to medication. Compliance Tip of the Month
The Compliance tip of the month is focused on the standard/requirements regarding establishment of an antibiotic stewardship program. Specifically, regarding a lack of evidence that the organization is collecting, aggregating, analyzing and acting in response to data including adherence to antibiotic prescribing guidelines.
Organizations are expected to provide evidence that the monitoring is occurring and that it is incorporated into the QAPI program. The intent behind the regulation is to monitor and improve the use of antibiotics in accordance with nationally recognized guidelines. Read more on the CIHQ newsletter.
ARS Regulatory Alerts
CIHQ-ARS notifies member organizations of any new or modified accreditation standard, or CMS COP for acute care hospitals. Each alert summarizes the standard / regulation, likely impact to members, and recommends compliance strategies. Whenever possible, alerts are limited to one-page in length so that they can be quickly read and disseminated. Topics for this month’s alerts are:

The EHR build is now majorly back on track and hoping for a late summer go-live. We have so many CIPs and improvements to our current building coming up! These are such exciting times!
