You're an intern doing your first overnight in Area D. EMS triage calls to tell you they're sending over a 40 year-old male BIBEMS after he was found running naked through the streets. On arrival he is extremely agitated, cursing and kicking in the stretcher. He has vomited on himself.
What should you do?
While you may eventually end up doing this, it is better to attempt verbal de-escalation prior to sedation so you can obtain a better history from the patient.
This may eventually be necessary to ensure the safety of the patient and staff, however verbal de-escalation should be attempted first.
Verbal de-escalation can be very effective with patients who are agitated but cooperative. Use a calm, non-confrontational approach and provide comfort with blankets, food, and drinks (avoid giving the patient anything that can used as a weapon such as a soda can).
This is always the right answer. +IV, O2, monitor.
