Bugfixes

#0124 - When a EPUB’s TOC is incomplete and inconsistent with the spine sequences, its text will not be extracted even when “all chapters” are chosen.

#0125 - When individual chapters are selected from text extraction, their full content will not be extracted. This happens when their text is split into multiple xhtml files.
